US visa: how to correctly indicate a certified translation?

I’m preparing documents for a US visa and don’t understand how to properly prepare the translation. Do they just write that the translation was done by a certified translator — and that’s it? Or do you also need a stamp, a notary’s signature, or some special form? I’d be grateful if you could share how you did it.
