Does anyone know — if another dependent appears during administrative processing, does that mean the primary visa (L1/H1B) needs to be reapplied for? The original application did list dependents, so I don’t understand whether everything will have to be redone because of this.
No — don’t touch the primary. The dependent files a separate DS-160 and has their own interview. AP (administrative processing) for the main case goes on its own track.

The dependent is stored in a separate file; you don’t touch the primary, even if the AP is running in parallel.

Don’t worry so much, you don’t need to reapply for anything. The dependent goes its own way — a separate DS-160 and their own interview; don’t touch the main one. I remember that panic myself when it seems like everything is falling apart, but in reality everything gets sorted out calmly )
